Endobronchial metastases from extrathoracic primary malignancies are uncommon. A postmortem study revealed that endobronchial metastases were present in only 2% of patients with solid tumors. Breast, colorectal, and renal cell carcinomas are the most common primary tumors. Usually endobronchial metastasis does not occur until a long time after establishing the diagnosis of the primary malignancy. We present a rare, interesting case of multiple endobronchial metastases from renal cell carcinoma arising from the horseshoe kidney, with initial symptoms that resulted from multiple endobronchial metastases-related pulmonary symptoms leading to the diagnosis of renal cell cancer.
