We present the case of a 32 year old man suffering from acalculous cholecystitis due to polyarteritis nodosa. He was admitted to hospital with right upper abdominal pain and high fever. Abdominal ultrasonography revealed acalculous cholecystitis, and cholecystectomy and appendectomy were performed. Pathologic examination of both appendix and gallbladder showed involvement of medium size arteries due to polyarteritis nodosa. Surgeons should be aware of this rare cause of acute acalculous cholecystitis.
