Özet

AUTOMATED EXTERNAL DEFIBRILLATOR USE IN PUBLIC AREAS AND ITS IMPORTANCE FOR PUBLIC HEALTH

Out-of-hospital cardiac arrest (OHCA) is a cause of high mortality and is an important public health problem. On average, 70-90% of OHCA cases are lost before reaching the hospital. However, it is stated that when cardiopulmonary resuscitation (CPR) and automatic external defibrillator (AED) are used, the survival rate is as high as 75%. While the survival rate is 29% when CPR is started within the first four minutes, the survival rate drops to 7% when started after four minutes. If immediate intervention is not made when cardiac arrest develops, neurological and functional recovery weakens with each minute and the probability of survival decreases. AED devices are safe and effective systems that analyze the heart rhythm in cases of cardiac arrest, determine that an electric shock is necessary, and provide electric current to the heart to ensure normal heart function, even when used by volunteers. The benefits of defibrillation programs accessible to the public and AEDs being more accessible and available in society are indisputable. These benefits have been attributed to the reduction in time to defibrillation by witnessed rescuers, as survival in shockable AED decreases significantly with each minute of delay in defibrillation. This can only be achieved with public access programs and on-site AEDs. When the nearest AED is accessible in AED, 30-day survival is almost doubled compared to when it is not. In many countries around the world, the availability of defibrillators in public spaces is regulated by various legislations. As nurses, we should work to promote the use of AEDs in public spaces, schools, sports centers, and crowded areas in Turkey, and to ensure that they are included in legislation and guidelines, as part of our advocacy role for improving public health.
