Özet

Objective: The aim of this study is to determine the relationship of sperm morphology with fertilization ratio and embryo quality in ICSI cycles and also the relationship of sperm morphology with sperm DNA damage. Materials and Methods: Fifty six infertile couples were included in this prospective study. Azospermic patients and women over the age 35 were excluded from the study. Controlled ovarian hyperstimulation was applied in the context of long protocol, using gonadotropin releasing hormone analogue (GnRHa) and after the second day of her menses, recombinant gonadotropin (r-FSH) or human menapousal gonadotropin (hMG) was applied. After the administration of 10.000 IU hCG IM, oocytes were collected by the transvaginal route. Sperm morphology was evaluated in preparates dyed using Diff-Quick method, using Kruger strict criteria. If more than 4% of spermatocytes had normal morphology then that patient was included in the normal group. Acridine orange dye was used to determine the sperm DNA damage and if >56% green or <56% red fluorenscence was evident then that sample was included in the AO(-) and the others in the AO(+) group. Fertilization was confirmed by the presence of 2 pronuclei and polar body at 16-18 hours. The quality of embryos were evaluated in the third day and due to their being fragmented or even they were classified as grade I,II,III. Results: Fertilization ratio was 73.28%, in normal sperm morphology group and was 51.69% in abnormal morphology group. When the two groups were compared according to embryo quality, normal group had a mean 50.17 grade I, 37.19 grade II, 46.91 grade III embryo's. Abnormal group had a mean 53.50 grade I, 42.55 grade II, and 53.91 grade III embryo's. In the group stained normal (undamaged DNA) with acridine orange (AO) sperm morphology was 60.6% normal and 39.4% abnormal. In the group stained abnormal (damaged DNA) with AO, sperm morphology was 21.27% normal and 78.3% abnormal. Discussion: There is significant relationship between sperm morphology and DNA damage. It is shown by ICSI that sperm morphology has an effect on fertilization ratios but no relation was found between sperm morphology and embryo quality.
