Abstract

Despite the histamine releasing effects of opioid, allergic reactions were rare. Meperidine, a synthetic opioid, is widely used for premedication in preoperative period and for analgesia in postoperative period. Meperidine could cause direct degranulation in mast cells without specific IgE antibodies. Allergic reactions from local urticarial reactions to anaphylaxis could be seen. In this case, possible allergy mechanisms and treatment steps of synthetic opioid meperidine were discussed in which acute localized urticarial reactions were observed with meperidine administration due to postoperative shive-ring.
