Özet

In order to determine the accuracy of computed tomography in the staging of esophageal and gastroesophageal junction carcinomas, computed tomography results were compared with operative and pathologic findings in 40 patients. Computed tomography examinations were evaluated with respect to nodal metastases, adjacent spread, and hepatic metastases. 26 of the cases were operable, 14 were inoperable according to the preoperative computed tomography findings. Curative resection was performed to the 26 cases, and palliative surgery was performed to the seven inoperable cases. Sensitivity of computed tomography was 66% for nodal metastases, 75% for adjacent spread, 80% for hepatic metastases, whereas the specificity and positive predictive value were 100% for all three parameters for the 33 cases all of which underwent surgery. The negative predictive value was less than 77% for nodal metastases, 90% for adjacent spread and 96% for hepatic metastases. The accuracy were 91% for nodal metastases and adjacent spread, 97% for hepatic metastases. These results indicate that computed tomography is an accurate examination method for staging of the esophageal and gastroesophageal junction carcinomas and can prevent unnecessary surgery in patients with inoperable tumors.
