Tooth Extraction in the Anterior Esthetic Zone with Immediate Implant Placement and Provisional Restoration

Abstract

OBJECTIVE: There is an increasing trend toward immediate single-tooth implant placement in the

esthetic zone, preferably with an immediate provisional restoration. According to Slagter et al., immediate

implant placement with provisionalization results in slightly less peri-implant bone loss compared with

delayed provisionalization. Other studies indicate a slightly higher risk of implant loss in extraction

sockets; however, immediate implants better preserve peri-implant bone and are generally well accepted

by patients, likely due to the reduced trauma of minimally invasive flapless procedures.

CASE PRESENTATION: A 45-year-old female patient with no systemic disease presented to the

Department of Oral and Maxillofacial Surgery, Faculty of Dentistry, Gazi University, with pain in tooth #21.

Clinical and radiographic evaluation revealed external root resorption. Given her high esthetic

expectations, flapless immediate implant placement with provisionalization was planned. Under local

anesthesia, atraumatic tooth extraction was performed, and the implant was positioned to align with the

cingulum of adjacent teeth. Thin buccal bone was augmented using xenograft and a collagen membrane.

The extracted crown was modified and stabilized for provisional restoration. At four months, a PEEK

abutment and temporary prosthesis were applied to guide gingival shaping. At six months, soft tissue

contours and papillae were restored.

RESULT: Immediate implant placement with provisionalization reduces the need for a second surgery

and ensures an esthetic provisional, making it a preferred approach in clinical practice.
