Immediate Loading for the Rehabilitation of Edentulous Patients With the All-On-Four Concept

INTRODUCTION: In the all-on-four concept, which shows high success in long-term follow-up; In edentulous jaws, the posterior implants are placed distally inclined at an angle of approximately 30 degrees, and the anterior two implants are placed vertically or slightly inclined if needed. This treatment method is a method that can be loaded immediately with fixed full-arch prostheses on four implants. The aim of this presentation is to present the prosthetic treatment option with immediate loading on the implants made with the all-on-four concept in the maxillary and mandibular completely edentulous case. CASE REPORT: A 52-year-old fully edentulous male patient applied to our clinic for treatment. As a result of the clinical and radiographic examination, it was decided to apply the all-on-four concept and to perform immediate loading in order to avoid sinus surgery and to eliminate the need for grafting. The total prothesis was prapered before the implantation. Immediately after the implants were made, impression material was placed into the previously prepared prostheses and placed in the mouth again in order to determine the implant positions. After the temporary titanium abutments were attached, the prosthesis was placed and the holes were enlarged for passive fit. A ruber roof cover was attached to protect the sutures and isolate the oral environment. The prostheses was connected with temporary abutments, the screw entry points of which were closed with teflon tape. After the acrylic polymerization was completed, the screws were removed. The deficiencies around the abutments were completed and the teeth forming cantilevers were removed. The prosthesis were modified to be convex. The screw was torqued to the multiunits with 15 N/cm. CONCLUSION: With the all-on-four concept, immediate prosthetic loading is a treatment option that avoids extra surgical procedures, provides comfort to the patient with a small number of implants and rapid prosthetic rehabilitation.
