Özet
Objevtive: This study aimed to evaluate pregnancy outcomes in infertile women undergoing in vitro fertilization (IVF) treatment and to assess the independent impact of serum anti-Müllerian hormone (AMH) levels on these outcomes. Material and Methods: A retrospective cohort study was conducted on 190 women who achieved clinical pregnancy following IVF and embryo transfer between January 2019 and December 2022. The study group included 92 women (48.42%) with polycystic ovary syndrome (PCOS), while the control group comprised 98 women (51.58%) without PCOS. Results: Women with PCOS had significantly higher miscarriage rates (37% vs. 12.2%, p<0.001), higher rates of hysteroscopic corrections for congenital uterine abnor-malities (19.6% vs. 7.1%, p=0.017), and lower live birth rates (58.7% vs. 86.7%, p<0.001) compared to non-PCOS women. Preterm delivery(<37 weeks) (27.8% vs. 10.6%, p=0.028) and complicated pregnancies (18.5% vs. 5.9%, p=0.025) were also more frequent in the PCOS group. Serum AMH levels were significantly higher in women with preterm delivery (5.3 ± 2.7 ng/mL vs. 2.5 ± 2.1 ng/mL, p<0.001). Multivariable analysis identified AMH level as an independent risk factor for preterm delivery (OR: 1.503, 95% CI: 1.201–1.881, p<0.001; adjusted for age, BMI, and congenital uterine anomalies). ROC curve analysis demonstrated good predictive performance, with an AUC of 0.821. Conclusion: Elevated serum AMH levels are an independent predictor of preterm delivery in ART pregnancies, regardless of PCOS diagnosis. Serum AMH levels may serve as a valuable tool for identifying high-risk preg-nancies, enabling more personalized and proactive management.
