Hyponatremia, a prevalent electrolyte disorder in clinical practice, often poses diagnostic challenges due to its diverse etiologies. The authors present a complex case of a 91-year-old patient with per-sistent hyponatremia and postprandial hypoglycemia. Despite dietary adjustments, the hyponatre-mia remained refractory, prompting further investigation. Subsequent diagnostic imaging revealed a pituitary macroadenoma, shedding light on the underlying cause of the electrolyte imbalance. This case underscores the rarity of hyponatremia caused by hypopituitarism due to macroadenomas, highlighting the importance of recognizing this uncommon etiology.
