Removal of root canal sealers using passive ultrasonic irrigation and photon-induced photoacoustic streaming during retreatment: a micro-CT study

Objective
The aim of this study is to compare the effectiveness of passive ultrasonic irrigation (PUI) and photon-induced photoacoustic streaming (PIPS) in removing residual filling materials during retreatment of root canals filled with epoxy resin–based and tricalcium silicate–based sealers using micro-computed tomography (micro-CT).
Materials and methods
Twenty-eight extracted mandibular premolars with a single root canal were randomly divided into two groups according to the sealer used (n = 14): resin-based AH Plus and tricalcium silicate–based AH Plus Bioceramic. Root canals were obturated using the single-cone technique and stored for 30 days. Retreatment was performed with the ProTaper Universal Retreatment (PTUR) system, after which each group was further subdivided according to the irrigation activation method: PUI or PIPS (n = 7). Micro-CT scans were obtained after obturation, mechanical retreatment, and irrigation activation. The volume of residual filling material was calculated and statistically analyzed.
Results
Residual filling material was detected in all specimens. No significant difference was observed between AH Plus and AH Plus BC sealers in terms of residual filling material removal (p > 0.05). Both PUI and PIPS significantly reduced the volume of remaining filling material compared with mechanical retreatment alone (p < 0.05). However, no significant difference in removal efficiency was found between the two activation techniques (p > 0.05).
Conclusions
Within the limitations of this in vitro study, irrigation activation improved the removal of residual filling materials following mechanical retreatment; however, complete removal was not achieved, and no significant difference was observed between PUI and PIPS.
Clinical relevance
Irrigation activation techniques such as PUI and PIPS may enhance the removal of residual filling materials during endodontic retreatment; however, complete elimination of obturation materials remains challenging.
