A case of spontaneous rupture of a right intrahepatic bile duct with subdiaphragmatic bilioma formation treated by percutaneous drainage, endoscopic sphincterotomy and extraction of a large common bile duct stone is presented. A 74 years old woman was admitted with dyspnea, abdominal pain, and jaundice. She had undergone a cholecystectomy operation 16 years ago. Ultrasound and computed tomography revealed a massive fluid collection in the subdiaphragmatic area. Endoscopic retrograde cholangiography demonstrated extravasation of the contrast medium from a distal biliary radicle in the right lobe of the liver and a large stone in the common bile duct. The bilioma was successfully treated by percutaneous drainage, endoscopic sphincterotomy and removal of the common bile duct stone. The bilioma healed within seven days of treatment and endoscopic retrograde cholangiography showed resolution of intrahepatic bile duct leakage. Nontraumatic biliomas are very rare and only 25 cases of spontaneous bilomas have been reported in the to date literature. Endoscopic treatment can be recommended as a safe and effective alternative method for patients with spontaneous bilioma secondary to damage of biliary tree and bile leakage.
