A retrospective review of 30 patients with dystonia or Parkinson's disease that were treated surgically at the Department of Neurosurgery of Istanbul Faculty of Medicine is presented. The dystonia was primary in 11 patients and secondary five patients with primary in 11 patients and secondary in 13 patients. Five patients with primary dystonia and 10 patients with secondary dystonia had generalized dystonia. These 30 patients underwent 45 computed tomography guided ventrolateral thalamotomy or pallidotomy procedures. There were no significant complications related to surgery. The follow-up time-up was 6 months to 3 years. Clinical evaluations were made pre- and postoperatively with dystonia movement and disability scales, and the Modified Hoehn and Yahr scale. Evaluation 1 month after surgery showed marked improvement in 91 % of primary dystonia patients, and in 61 % of secondary dystonia patients. Most of this amelioration was lost by the end of the first year in primary dystonia patients. None of the three patients with secondary focal dystonia obtained benefit from surgery. Parkinson disease patients obtained significant and lasting benefit.
