Immune privilege is described by Peter Medawar and colleagues 70 years ago for describing the
immunological situation of some vital organs for transplantation. Testis, central nervous system
(CNS) and anterior camera of the eye are immune privilege sites. Testicular immune privilege is
important for protection of male fertility. Primary testis lymphomas are very rare tumors. Most of
them are diffuse large B cell type. Most frequently they represent activated B cell phenotype and
similarly with the other immune privilege site lymphomas their behavior is aggressive. Primary
testicular large B cell lymphomas most frequently tend to show recurrence on contralateral testis
or CNS. There are discussions on their treatment options. Last studies revealed several mutations
which may play key role on disease development. The molecular profile of the tumor is
important for guiding the treatment options. The mutational status could be helpful for tailoring
the treatment and getting success on controlling the disease. This review article is prepared to
sumarize the immune privilege concept as well as the histopathological, immunophenotypic
and molecular characteristics of the primary testicular lymphomas representing one of the
immune privilege site lymphomas.
