Comparison of the Results of Open Repair and Laparoscopic Repair With the Pirs Method in the Treatment of Hydrocele in Children

Özet

Introduction: Hydrocele is one of the most common inguinoscrotal

surgical pathologies in boys. Formation of the hydrocele in children is

a result of the transfer of the intraperitoneal fluid via a patent

processus vaginalis(PPV) into the scrotum. While the traditional

approach to pediatric hydrocele has been open repair(OR) for years,

it has also impacted hydrocele with the worldwide adoption of

laparoscopic repair(LR) of inguinal hernia.The aim of this article is to

compare the outcomes of both techniques in a single center with a

special emphasis on recurrence.

Patients and Methods: We retrospectively analyzed the clinical data

of all the patients who underwent hydrocele repair from August 2016

to November 2022. The standard approach was OR in hydrocele until

the November of 2021 and after that the approach was switched to

LR regardless of any condition except the consent of the patient.In

the LR group, single port percutaneous internal ring suturing was

performed.

Results:102patients(OR=74; LR=28) included in study.The overall

average follow-up period is 28,5months. It was 50,5months in open

group, 6,6months in laparoscopic group. On the OR group, there

were 5recurrences, 2wound infections and 5hematomas. In the

laparoscopic group there were 1recurrence and 1hematoma. In

50patients in open group, it was observed that inguinal ring was

open, in 5patients PPV was found and in 19patients there was no

connection with abdominal cavity. In 3patients whose inguinal ring

was observed normal, recurrence was occured. In 25patients in

laparoscopic group, it was observed that inguinal ring was open, in

2patient PPV was found, in 1patient there was no connection with

abdominal cavity and contralateral methachronus hernia was

observed in 4patients.

Conclusion: Laparoscopy may reveal intrabdominal connection of

hydrocele better than open approach. It provides high quality view of

both inguinal rings and has the advantages of minimally invasive

surgery.
