Özet
Background: To demonstrate a fertility-preserving laparoscopic laser ablation technique for ovarian endometrioma and to evaluate its short-term clinical and ovarian reserve outcomes. Laser ablation aims to minimize damage to healthy ovarian tissue while effectively treating endometriotic lesions.
Methods: A 41-year-old patient with chronic pelvic pain and ovarian endometrioma underwent laparoscopic laser ablation after failure of multiple medical treatment modalities. The procedure was planned as a fertility-preserving intervention. Following cyst drainage and inspection of the cyst cavity, selective laser vaporization of endometriotic tissue was performed while minimizing thermal spread to the surrounding ovarian cortex. Preoperative and postoperative ovarian reserve was assessed using serum Anti-Müllerian Hormone (AMH) levels.
Results: The procedure was completed laparoscopically without intraoperative or postoperative complications. Significant symptomatic improvement was achieved, with dysmenorrhea visual analog scale (VAS) score decreasing from 9 preoperatively to 3 at 3-month follow-up. Ovarian reserve was largely preserved, with AMH levels showing a limited decline from 2.97 ng/mL preoperatively to 2.52 ng/mL postoperatively.
Conclusions: Laparoscopic laser ablation may represent a fertility-preserving alternative to cystectomy in selected patients with ovarian endometrioma, particularly in cases with diminished ovarian reserve or bilateral disease. The precise and superficial tissue effect of laser energy may help reduce ovarian damage while maintaining satisfactory clinical outcomes.
