Using The All-On-Four Concept to Rehabilitation of the Edentulous Jaws With Surgical Perspective

Özet

INTRODUCTION: The allonfour implant design is an implant application technique that is applied in severely resorbed completely edentulous maxilla and mandible. Implants are placed in the premaxillary region in the maxilla and in the interforaminal region in the mandible. Although it is widely used in the mandible, this method could not be fully evaluated in the maxilla. CASE REPORT: Case 1: A 52 old male patient applied our department for edentulous maksilla and mandible. We consulted with prosthodontist and decided to using all-on-four concept for rehabilitation of both jaw. We took care of interincisal distant, pnomatisation of maxillary sinus and alveolar defect of mandibular posterior bone when deciding the operation. First, it was placed posterior angled implant, then the anterior implant was placed. It was used agressive implants for obtaining the primer stabilization. It was taken care do not perforate maxillary sinus when drilling the posterior hole at maxilla, therefore, opened a small window at lateral of the sinus bone. The guide was used to capture the angle of 30 degrees. While performing the posterior implant in the mandible, it was started over the mental foramen. Posterior implants were placed at an approximately 30 degree angle. The anterior implants were then placed at a straight angle. Case 2: A 71 old male patient applied our department for partial edentulous maxilla and mandible. After the consultation with the prosthodontist, we decided to using the all-on-four concept in the maxilla. Since the patient has healthy canine-canine teeth in the mandible, we decided to make a fixed traditionally prosthesis with implants placed in the posterior regions. The maxillary all-on-four concept was made following the principles of the previous case and loaded immediately. CONCLUSION: The all-on-four concept is a contemporary, quick and less invasive technique for rehabilitation of jaws. Therefore, it has become more popular.
