Karbon ayak izi, ekolojik ayak izinin en önemli
bileşenidir ve sürdürülebilir kalkınmanın önemli bir unsurudur. Sağlık
hizmetlerinin sunumundan kaynaklanan sera gazı emisyonları, birçok ülkede
ulusal emisyonların önemli bir bölümünü temsil etmektedir. Ülkeler karbon ayak
izlerini azaltmak için çeşitli yöntemler geliştirmektedir. Bu yöntemlerden biri
de sağlık hizmetlerinin teknoloji destekli uzaktan sunumudur. Bu derlemenin
amacı, sağlık hizmetlerinin teknoloji destekli uzaktan sunumunun (tele-sağlık)
çevresel etkilerine ilişkin literatürü incelemektir. Bu amaçla, hem uzaktan
sağlık hizmeti sunumu hem de ekolojik ayak izinin önemli bir boyutu olan karbon
ayak izinin azaltılması ile ilgilenen araştırmacılara literatürün sistematik
bir içerik analizi sunulacaktır. Bu araştırma için Web of Science veri
tabanında araştırma amacına uygun anahtar kelimeler kullanılarak arama yapılmış
ve ulaşılan çalışmalar içerik analizi kullanılarak analiz edilmiştir. Sistematik incelemede PRISMA prosedürleri
takip edilmiş ve son aşamada 21 araştırma makalesi elde edilmiştir. İnceleme
sonucunda araştırmaların çoğunun İngiltere kaynaklı olduğu, en çok yayın kabul
eden dergilerin "Journal of Telemedicine and Telecare" ve
"Annals of The Royal College of Surgeons of England" olduğu ve yayın
yılı bakımından en çok araştırmanın 2022 yılına ait olduğu tespit edilmiştir.
Çalışmaların neredeyse tamamının uzaktan sağlık hizmeti sunumunun ulaşım
boyutuna odaklandığı ve seyahat mesafesi ile karbon emisyon tasarrufunu
araştıran çalışmalar olduğu bulunmuştur. Yapılan içerik analizi ile
araştırmaların büyük oranda, evde bakım hizmetleri ve e-sağlık süreçleri ile
başta ulaşım olmak üzere enerji verimliliği ve hizmet süreci esnasında yapılan
bireysel tüketim süreçlerine odaklandığı tespit edilmiştir. Özellikle COVID-19
pandemi süreci sonrasında sağlık sistemlerinin daha bilinçli bir şekilde
tele-sağlık uygulamalarına yöneldiği görülmektedir. Buna paralel olarak karbon
ayak izine ait çalışmaların 2022 yılında yoğunlaştığı, bu alanda duyarlı
ülkelerin başında ise İngiltere olduğu gözlenmiştir. Henüz sınırlı düzeyde
olduğu görülen alan yazında gelecek araştırmalar için karbon ayak izinin
azaltılması konusunda sağlık sektöründe kelebek etkisinin nasıl
yaratılabileceği, bireysel düzeyden başlayarak kurumsal düzeye doğru proaktif
bir davranışın nasıl geliştirilebileceğinin araştırılması önerilmektedir.
Yaşanabilecek şimdilik tek bir tane dünyanın olduğu varsayıldığında, gelecek
nesiller adına bu alanda yapılacak her çalışmanın önemli olacağı
düşünülmektedir.

The carbon footprint is the most important component
of the ecological footprint and an important element of sustainable
development. Greenhouse gas emissions from the delivery of health services
represent a significant portion of national emissions in many countries.
Countries are developing various methods to reduce their carbon footprint. One
of these methods is technology-enabled remote delivery of healthcare services.
The aim of this review is to examine the literature on the environmental
impacts of technology-enabled remote delivery of healthcare services
(telehealth). To this end, a systematic content analysis of the literature will
be presented to researchers interested in both remote healthcare delivery and
carbon footprint reduction, which is an important dimension of ecological
footprint. For this research, the Web of Science database was searched using
keywords appropriate to the research purpose and the studies were analyzed
using content analysis. PRISMA
procedures were followed in the systematic review and 21 research articles were
obtained in the final stage. As a result of the review, it was determined that
most of the studies originated from the United Kingdom, the journals that
accepted the most publications were "Journal of Telemedicine and
Telecare" and "Annals of The Royal College of Surgeons of
England", and the most studies in terms of publication year belonged to
2022. It was found that almost all of the studies focused on the transportation
dimension of remote healthcare delivery and investigated travel distance and
carbon emission savings. With the content analysis, it was determined that the
majority of the studies focused on home care services and e-health processes,
energy efficiency, especially transportation, and individual consumption
processes during the service process. Especially after the COVID-19 pandemic
process, it is seen that health systems are more consciously turning towards
telehealth practices. In parallel with this, it was observed that the studies
on carbon footprint intensified in 2022, and the UK is one of the most
sensitive countries in this field. In the literature, which is still at a
limited level, it is recommended to investigate how a butterfly effect can be
created in the health sector in terms of reducing the carbon footprint, and how
a proactive behavior can be developed from the individual level to the
institutional level. Assuming that there is only one world to live in for now,
it is thought that every study in this field will be important for future
generations.
