Introduction: Hypersensitivity pneumonitis (HP) also known as extrinsic allergic alveolitis is a chronic, immunologically mediated inflammatory disorder that is caused by the diffuse and repeated inhalation of antigens, such as organic particles of animal or herbal origin and certain chemical compounds. Agents capable of inducing HP are found in a variety of settings including the home, workplace and in recreational activities. Drug options for the treatment of chronic HP are limited. Case presentation: The present study reports the case of a patient who was diagnosed with chronic HP when working as a cook. The patient was treated with azathioprine after experiencing severe side effects of methylprednisolone therapy. Conclusion: Corticosteroids are the most commonly recommended treatment. However, severe side effects of prolonged steroid therapy are an important limitation. In chronic HP, azathioprine therapy is effective in cases who are unresponsive to or have serious adverse effects with steroid therapy.
