The problem-oriented medical system is a tool which provides a systematic approach to patient care by providing clear, concise, and structured statements about medical actions. The focal point of the system is the problem-oriented medical record. This record system enhances continuity of patient care and allows one to document outcome of patient care. The problem-oriented medical system allows all individuals involved in a patient's care to determine the importance and relevance of their activities. Except for the timing and integration of various medical actions, the problem-oriented medical system does not dictate the type of information gathered about a patient or methods of problem management. The problem-oriented medical system is most useful to those who comprehend the concepts upon which it is base, and who are familiar with the mechanics of the problem-oriented medical record.
