Abstract
Cytokines are proteins that regulate immune and inflammatory reactions as well as haematopoiesis and remodelling. The balance between levels of cytokines, their receptors and specific inhibitors controls inflammatory reactions in tissues. Their multiple, overlapping and sometimes, contradictory functions depend on their local concentration, and the presence of other cytokines and mediators. A wide range of cellular responses are effected or triggered when cytokines interact with cells. These include inflammation, angiogenesis, immunomodulation and extracellular matrix deposition. With respect to inflammation, proinflammatory cytokines are classified into those involved in Th1 responses, such as interleukin IL-1, IL-6, IL-12 and TNF- and those involved in Th2 responses including IL-4, IL-5, IL-10, IL-13. Several of these cytokines are also involved in down-regulation of inflammatory pathway such as IL-10 and the IL-1 receptor antagonist (RA). The complex interactions of cytokines is referred to as the cytokine network, and are being defined as it relates to inflammatory processes. Cytokines that play a critical role in regulating and perpetuating inflammation in lung disorders, and several specific cytokine inhibitors are now in development for the future therapy of these diseases. This review presents the evolving concepts of the involvement of cytokines in the initiation or progression of inflammation in chronic lung diseases. How these extracellular signaling proteins are involved in such lung disorders as asthma, COPD, silicosis and asbestosis and therapeutic approaches are also discussed.
