Adie tonic pupil is a neuro-ophthalmological disorder characterized by a dilated pupil with poor response to light. While most cases are idiopathic; autoimmune diseases, vascular diseases, autonomic disorders, and infections such as COVID-19 have been found to be associated with Adie tonic pupil. We present a 39-year-old woman who developed unilateral Adie tonic pupil following COVID-19. Diagnosis was confirmed by pupillary constriction with dilute pilocarpine test, indicating denervation hypersensitivity. This case adds to the growing evidence of post-COVID-19 neurological complications and underscores the importance of considering Adie tonic pupil in patients presenting with anisocoria after viral infections, especially in the context of the COVID-19 pandemic.
