A New Perspective on Abdominal Obesity Assessment in Older Individuals: Anthropometric Ratios to Age

Aim: To determine which anthropometric measurement most strongly associated with metabolic syndrome (MetS) in older adults and to investigate the value of anthropometric ratios to age (AnRAge) in demonstrating abdominal obesity. Methods: The study included 107 patients. Waist circumference/age (WC/A), hip circumference/age (HC/A), waist-to-hip ratio/age (WHR/A), and body mass index/age (BMI/A) were calculated. Visceral fat thickness (VFT) was measured by ultrasonography (USG). The correlation between anthropometric measurements and AnRAge (WC/A, HC/A, WHR/A, BMI/A) with VFT was examined. Results: A positive correlation was found between VFT with systolic blood pressure, fasting blood glucose, triglyceride, waist circumference (WC), hip circumference, waist hip ratio, body mass index, WC/A, HC/A, WHR/A, BMI/A, while a negative correlation was found with HDL cholesterol. Also, the anthropometric measurement with the highest correlation coefficient with VFT was WC (r =.727; p <.001). Additionally, VFT was shown to have a higher correlation coefficient with AnRAge than with anthropometric measurements, and WC/A was found to have the highest correlation coefficient (r =.762; p <.001). When the relationship between MetS criteria and other related factors with visceral obesity was examined, only MNA-SF (OR: 2.238 (1.149–4.360), p =.018) and WC/A (OR: 365165.088 (12.191–10938004890.391), p =.015) were found to be significantly associated with visceral obesity. The optimal cut-off value of WC/A to predict abdominal obesity in females was measured as ≥ 1.3750 cm/year (83.3% sensitivity and 81.1% specificity) and in males was measured as ≥ 1.3116 cm/year (72.7% sensitivity and 65.2% specificity). Conclusions: We confirmed the association of visceral obesity with MetS and anthropometric measurements in old patients, and found that MetS was better associated with WC in older adults. Furthermore, the stronger association of AnRAge with abdominal obesity and MetS, rather than anthropometric measurements, was a novel finding. However, to determine whether AnRAge is more valuable in identifying abdominal obesity and MetS, multicenter studies with larger sample sizes are needed.
