The authors present the technique of computerized tomography (CT)-guided percutaneous trigeminal tractotomy (TR)-nucleotomy (NC) with a special electrode system. The present study included 30 patients: 7 patients with atypical facial neuralgia, 8 with pain caused by craniofacial malignancies, 6 with glossopharyngeal neuralgia, 3 with, geniculate neuralgia, 3 with failed trigeminal neuralgia, 2 with postherpetic neuralgia, and 1 with anesthesia dolorosa, Complete or satisfactory pain control was obtained in 24 patients (80%). The best results were obtained from the patients with glossopharyngeal neuralgia and geniculate neuralgia. The TR-NC showed promising results in craniofacial malignancies and atypical facial neuralgia. Postoperative temporary ataxia was the only complication and was observed in 4 patients. The indications for CT-guided percutaneous trigeminal TR-NC are almost the same as for the nucleus caudalis dorsal root entry zone operation, but the effectiveness of the procedure is sufficiently high and the complication rate is extremely low. With its advantages of minimal invasiveness, high success, and low complication rates, the percutaneous CT-guided TR-NC should be considered as a rational alternative to other pain-relieving procedures.
