Although methemoglobinemia is a rare clinical condition, it causes hazardous effects unless it is treated in a proper ways in early period. Methemoglobin results from oxidation of ferrous iron (Fe2+) of the hemoglobin molecule to ferric iron (Fe3+), It is not difficult to diagnose methemoglobinemia in clinically suspicious conditions. In this report, four methemoglobinemia cases depended on local anesthetic toxicity, occurred in Orthopedic surgery theater in the last 10 years are reported and these cases arc discussed by looking over the published data from the literature.
