The differential diagnosis and treatment of a 2-year-old boy with a left-sided cystic neck bulge enlarged in size while straining, coughing, and crying is presented. Contemporary diagnostic tools were of little value, whereas its location, variability in size, and consistency at physical examination played a major impact.
