OBJECTIVE: The aim of this prospective study is to compare the effect of oxytocin versus misoprostol on Doppler resistance indices of the uterine and umbilical artery during labor induction. STUDY DESIGN: Seventy pregnant women with indications for induction of labor assigned to receive either intravenous oxytocin or misoprostol 50 μg per vagina every 6 h as needed. Doppler velocimetry of umbilical and bilateral uterine arteries were performed before and three hours after administration of oxytocin or misoprostol and in active phase of labor. In these arteries, pulsatility index (PI), resistance index (RI) and systolic/diastolic ratios (S/D) were measured. RESULTS: Demographic data of the two groups were comparable. In the latent and active phase of labor, PI, RI and S/D ratios in the umbilical artery were not change. These indices in evaluation of the uterine arteries were significantly increased in latent phase compared with those measured before labor induction with oxytocin or misoprostol. There were no significant changes of PI, RI and SID ratios of all arteries between latent and active phase of labor. The measured values of Doppler parameters of uterine arteries in misoprostol group were significantly higher than those in oxytocin group during the active phase, but they were similar in evaluation of the umbilical artery. Significant positive linear correlation was observed between high SID ratio in the umbilical artery before induction and the increased risk of caesarean section (r=0.282, p<0.05). CONCLUSION: Uteroplacental resistance increases in labor induction with oxytocin or misoprostol, but umbilical circulation is not affected. The high degree of S/D ratio in the umbilical artery before induction may be indicating the increased risk of caesarean section.
