Objective:The aim of this study is to emphasize the critical role of achieving both apical and coronal sealing in the success of endodontically treated teeth with extensive structural loss, and to evaluate the contribution of appropriate restorative approaches—particularly endocrowns— to the long-term functional and structural sustainability of such teeth.

Case Presentation:A 19-year-old female patient presented to the Faculty of Dentistry at Ankara University with complaints of recurrent swelling and fistula formation around the lower right first molar (tooth #46). Clinical and radiographic evaluations led to a diagnosis of necrotic pulp and subacute apical abscess. A multi-visit root canal treatment was initiated. In the first session, irrigation and preparation protocols were performed, and calcium hydroxide was placed as an intracanal medicament. At the second visit, symptoms had subsided, and the canal treatment was completed with gutta-percha and a sealer compatible with the previous preparation. Due to the patient needing to leave town, the endocrown restoration was postponed for three months after temporary restoration. A follow-up radiograph revealed a reduction in the size of the lesion. The necessary tooth preparation was performed for the endocrown, followed by intraoral scanning (CEREC Omnicam). The restoration was fabricated using CAD/CAM technology with a silicate ceramic block (CEREC BLOCK). After verifying the fit, the endocrown was cemented using an appropriate adhesive protocol and a dual-cure resin cement. Excess cement was removed, completing the treatment.

Conclusion:At the 10-month follow-up, periapical radiography confirmed complete healing of the lesion. No pathological findings were observed during the intraoral examination. The applied endocrown showed optimal adaptation with the tooth; no fractures or cementation- related complications were detected. The patient reported satisfaction with the treatment. Endodontically treated teeth tend to become more fragile due to various factors. Therefore, preserving tooth structure and ensuring restorative integrity are crucial for the prognosis of such teeth.
